  • What does success look like for producing and critically appraising routine treatment plans? What are the criteria for a clinically acceptable and optimal treatment plan e.g., dose coverage, OAR sparing, deliverability? How will you evaluate the plan’s quality, considering patient-specific needs and treatment goals across various anatomical sites?
  • What is your existing knowledge of treatment planning algorithms, dose calculation, plan optimisation, and evaluation metrics e.g., DVHs? What challenges might you face in optimising plans or identifying suboptimal areas, and how will you approach refining them? When should you seek expert advice on complex planning scenarios or challenging appraisals? How do you feel about the creative and critical aspects of treatment planning?
  • Which specific skills in treatment planning software, dose-volume histogram analysis, or critical thinking for plan appraisal do you aim to develop? What insights do you hope to gain regarding the trade-offs in plan optimisation and the factors influencing clinical acceptability across different patient cases?
  • Have you reviewed feedback from previous planning exercises or considered different planning techniques that might be relevant? What important information, such as clinical prescriptions, patient images, and departmental planning guidelines, should you review?

    • Difficulty in achieving prescribed dose coverage to target volumes while respecting OAR constraints.

    • Challenges in ensuring the plan is clinically deliverable with available machine capabilities.

    • Do you feel positive you can reach a successful conclusion, creating a high-quality plan?
  • You identify how you work within your scope of practice to address the unexpected development. Do you ensure the plan is robust and meets clinical requirements? What are you learning in the moment as a result of the unexpected development e.g., the impact of specific planning parameters, strategies for balancing competing objectives, or the limitations of planning algorithms.

  • Begin by summarising the key points of your experience producing and critically appraising routine treatment plans for various anatomical sites. Consider specific events, actions, or interactions which felt important during plan production and appraisal, including your own feelings at the time. Include any ‘reflect-in-action’ moments where you adapted to the situation as it unfolded during plan production and appraisal.
  • Identify what learning you can take from this experience of creating and appraising treatment plans e.g., what strengths did you demonstrate? What skills and/or knowledge gaps were evident? Compare this experience against previous engagement with similar treatment planning activities – were any previously identified actions for development achieved? Has your practice improved in creating and appraising treatment plans?
