Aim of this module
This module provides trainees with an advanced working knowledge of the technical applications of advanced monitoring using technological systems in critical care. The trainee will acquire the necessary skills to perform each of these.

- Explain possible complicating pre-analytical errors for abnormal blood gas results – including incorrect handling and pre-analysis sample maintenance.
- Troubleshoot questioned accuracy of abnormal arterial blood pressure.
- Perform bedside troubleshooting of an invasive blood pressure monitoring catheter and communicate corrective steps to the multidisciplinary team.
- Troubleshoot application of monitoring electrodes/sensors.

Learning outcomes
On successful completion of this module the trainee will be able to:
- Critically evaluate the application of POCT in the critical care environment.
- Explain the use of ventilation waveforms/flow loops in the diagnosis of a patient’s clinical respiratory condition.
- Critically evaluate the range of techniques and equipment required for monitoring in critical care, including invasive blood pressure management, resting energy expenditure and imaging.
- Evaluate the measurement principles underpinning common types of monitoring equipment used in critical care, including neurological monitoring.
Indicative content
- Application of investigations used in the management of the critically ill patient.
- The regulations, guidelines, protocols and procedures in place to manage the risk of pre and post analytical errors in POCT.
- Analytical aspects of sampling, and POCT analysis.
- Understand the principles of imaging techniques.
- Identify basic structures on radiological imaging, recognising examples of artefact.
- Describe the function of a selection of monitoring techniques, including transducers, and develop interpretation skills.
- The application of advanced monitoring in the management of common pathophysiology’s presenting in the critically ill patient.
- Describe the components of, and methodologies involved, in temperature measurement, including interpretation and application of therapies.
- The principles of the use of hyper/hypothermia units including accurate temperature monitoring.
- Understand the theory and clinical application of neurological monitoring, including bispectral index, NIRS and EEG.
- Describe the application of respiratory gas measurements related to the nutrition of the critically ill patient.
- Understand the advantages/disadvantages of measuring resting energy expenditure (VO2, VCO2, minute volume) as opposed to using calculated estimations.
Clinical experiences
Important information
Clinical experiential learning is the range of activities trainees may undertake in order to gain the experience and evidence to demonstrate their achievement of module competencies and assessments. The list is not definitive or mandatory, but training officers should ensure, as best training practice, that trainees gain as many of these clinical experiences as possible. They should be included in training plans, and once undertaken they should support the completion of module assessments and competencies within the e-portfolio.
Activities
- Visit the imaging department to observe imaging procedures such as x-ray, CT, MRI or ultrasound.
- Attend a POCT meeting to appreciate the governance procedures involved.
- Observe blood gas training for new users.
- Attend a meeting with an industry specialist to appreciate their role in research and development.
- Observe neurological monitoring.
