Details
Take a patient history for patients presenting with complex support needs from one of the following groups under supervision:
- Complex social issues
- Mental health diagnoses
- Dementia
- Learning disabilities
- Language or communication difficulties
Considerations
- Patient centred care and support
- History taking skills
- Safeguarding issues e.g. signs of female genital mutilation or suggestions of domestic violence or abuse
- The principles of dignity, privacy and confidentiality
- Signposting patients to access additional support where required
- Obtaining informed consent
