Aim of this module
This module will enable critical care trainees to identify the clinical signs and typical conditions associated with critical illness and resulting in admission to a critical care environment. Trainees will be able to perform techniques of advanced mechanical ventilation, managing, monitoring and interpreting to maximise clinical effectiveness and enhance safe patient care. Trainees will also be expected to perform a range of other life support and emergency resuscitation techniques. Trainees will be expected to build on the competence gained in the earlier module, building their professional practice.

Learning outcomes
The trainee should produce evidence of completion of basic or intermediate life support training before undertaking any of the learning outcomes.
- Perform the selection of the appropriate mode and pattern of ventilation with respect to the patient’s sedation level and lung pathology/pathophysiology, diagnosing common problems associated with ventilator testing and application.
- Set up, apply and manage a range of adjunct ventilation therapies, such as high frequency oscillatory ventilation (HFOV), high frequency jet ventilation (HFJV), nitric oxide therapy (NO), anaesthetic agent delivery in critical care and Heliox applications.
- Interpret and apply the monitoring techniques available on high specification ventilators with respect to the ventilation settings.
- Perform and manage Continuous Renal Replacement Therapy (CRRT) for a range of pathologies safely in accordance with current guidelines.
- Set up and operate an intra-aortic balloon pump (IABP) with reference to safety, risk management and therapeutic effects.
- Set up and manage a range of syringe drivers within the critical care environment.

Clinical experiences
Important information
Clinical experiential learning is the range of activities trainees may undertake in order to gain the experience and evidence to demonstrate their achievement of module competencies and assessments. The list is not definitive or mandatory, but training officers should ensure, as best training practice, that trainees gain as many of these clinical experiences as possible. They should be included in training plans, and once undertaken they should support the completion of module assessments and competencies within the e-portfolio.
Activities
- Observe the care and treatment of a range of patients within a critical care environment and discuss the procedures to manage risk to patients requiring critical care, including the underpinning evidence base with your training officer.
- Work within a range of theatres where patients who may develop or have a critical illness are treated, observing the work of the anaesthetic team and theatre team and reflect on the role of the multidisciplinary team in the care of the critically ill patient.
- Observe the practice of open heart surgery requiring cardiopulmonary bypass and discuss the principles and practice of clinical perfusion with a member of the clinical perfusion team.
- Observe the care of paediatric patients in a critical care environment and reflect on how this care may differ from that needed by adult patients.
- Attend a meeting at which the outcome of a clinical audit, research, innovation, or service development is presented and discuss with your training supervisor how evidence-based practice is implemented with respect to critical care.
